The Case Manager is a professional nurse empowered with the knowledge, responsibility, and accountability for managing patient care throughout the episode of illness to improve quality, cost, and service satisfaction outcomes. Complementary member of the health care team whose activity focuses on collaborating with other care givers and the patient/family to ensure: (1) services are well coordinated, appropriate, and timely, (2) resources are appropriately expended, (3) suitable and timely continuing care plans are implemented, (4) that an effective liaison relationship exists between physicians, the hospital, third-party payers, and utilization review companies and (5) that complex health-related problems are actively managed and resolved. The Case Manager demonstrates the ability to organize and carry out duties without direct supervision and has 24 hour, 7-day/week responsibility for his/her caseload of assigned physicians and patients. This position may provide services to patients of all ages; however, the primary populations served are adults and geriatric adults. Must possess excellent communications skills and must have demonstrated leadership skills. At least five years of clinical experience. Experience in continuing care planning, resource management desired. Experience in clinical assignment area strongly recommended. Registered Nurse licensed to practice in the State of Connecticut. Bachelors Degree in Nursing or enrollment in program required. Masters Degree preferred.
