· Performs bedside palliative care evaluation with patients and family.

· Assists with treatment and management of acute and chronic health conditions.

· Follow up on initial physicians consults, including ensuring orders, labs and radiology is performed.

· Assess pain level of patient and make appropriate plan of care.

· Support family, physicians and care team regarding patient care decisions.

· Work in collaboration with medical director to assist patient to reach health care goals.

· Consults with palliative care medical director as needed; as well as with other team members to gather information and educate and facilitate decision making of patient and family.

· Informs physicians and other team members of services provided.

· Provides on-going education of palliative care of staff.

· Assist in development of clinical practice guidelines/standards in support of quality palliative care.




Experience: Minimum of 3 years experience as a Staff Nurse. Minimum of 1 year Palliative Care experience. Must have or obtain a Connecticut state license.



Education: Graduate of an accredited school of nursing (Diploma, AD, BS, BSN)
